I. Your Protected Health Information (PHI)
This notice describes how medical information about you may be used and disclosed and how you can get access to this information. We are required by law to maintain the privacy of your PHI under the Health Insurance Portability and Accountability Act (HIPAA).
II. Special Protections for Substance Abuse Records (42 CFR Part 2)
Because we provide DOT SAP Evaluations, your records may be protected by Federal Law (42 CFR Part 2).
We cannot say to a person outside the program that you attend the program, or disclose any information identifying you as having a substance use disorder, unless you consent in writing, the disclosure is allowed by a court order, or the disclosure is made to medical personnel in a medical emergency.
III. How We Use Your Information
For Treatment: To provide, coordinate, or manage your health care.
For Payment: To bill you or your insurance company (BCBS, Aetna, Cigna) for services rendered.
For Healthcare Operations: To improve the quality of care and conduct business management.
IV. Your Rights
You have the right to inspect and copy your clinical record.
You have the right to request a restriction on certain uses and disclosures.
You have the right to receive a “Good Faith Estimate” of costs (as detailed on our Billing Page).
V. Complaints
If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer or with the Secretary of the Department of Health and Human Services.
